Individualized Needs Assessment
Bloomfield Social and Youth Services is staffed with a team of professionally trained social workers who are available to work with you and your family in determining your particular needs and the services and programs that are available to you.
Inter-Agency Referral Services
Often times the variety of organizations and agencies can be confusing and may make it difficult to know where to go for help. After we discuss your needs with you, we will work with you to make an action plan. The action plan will outline for you what steps to take to help you and your family. We can assist you with navigating through the services and programs available to you. If needed, we can make referrals to other agencies on your behalf.
